1. Small changes take far too long

When a change that should take a day takes a fortnight, and nobody can quite explain why, the delivery machine is clogged. It is usually accumulated technical debt, a brittle release process, or knowledge stuck in one person. Left alone, it gets slower, not faster.

2. The same problems keep coming back

If incidents recur, the same bug reappears, or the same firefight happens every few weeks, you are treating symptoms. Something underneath is unaddressed, and it is stealing time from building the things that grow the business.
